Skilled Nursing

Our registered nurses (RNs) and licensed vocational nurses (LVNs) provide medical care under the supervision of your physician. From medication management and patient education to monitoring complex conditions, our nurses deliver attentive, high-quality care that keeps patients safe and supported at home. We provide care for chronic conditions, post-operative recovery, medication administration, and more. Skilled nursing also includes vital sign monitoring, wound evaluations, IV therapy, and care coordination between patients, families, and physicians.

IV Therapy

As a part of our Skilled Nursing care, we provide intravenous (IV) therapy in the comfort of home—including hydration, antibiotics, and specialty infusions. Our nurses coordinate with referring physicians and specialty pharmacies to ensure treatment is safe, timely, and tailored to the patient’s plan of care. Home-based IV therapy can reduce hospital stays and allow patients to receive essential treatments in a familiar environment. We follow strict protocols for infection control and ensure all equipment and supplies are managed appropriately.

Wound Care

Also a function of Skilled Nursing care, wound care is one of our clinical strengths. Our skilled nurses manage surgical wounds, diabetic ulcers, pressure injuries, and more—using advanced techniques and close monitoring to reduce complications and support healing. We develop personalized wound care plans based on the patient’s diagnosis and risk level, and we work closely with physicians and specialists to promote healing and prevent infection. Proper wound care at home helps patients avoid ER visits and hospital readmissions. Our team follows strict protocols for proper wound care and equipment management to minimize the potential for infections.

Physical Therapy

Our licensed therapists help patients regain strength, mobility, and independence through personalized, goal-focused rehab—delivered right in the living room. Each treatment plan is designed to improve safety, reduce fall risk, and support long-term recovery after injury, surgery, or hospitalization. We help patients return to daily activities, rebuild confidence in movement, and maintain physical function—all without leaving home.

Occupational Therapy

We help patients reclaim their routines and confidence by improving their ability to perform everyday activities, from bathing to meal prep and more. Occupational therapy supports patients in adapting their environment, learning new techniques, and regaining independence in personal care and household tasks. It’s ideal for individuals recovering from stroke, joint replacement, or managing neurological or orthopedic conditions.

Speech Therapy

Whether recovering from stroke or managing a neurological condition, our speech therapists help improve communication, cognition, and swallowing abilities. We provide in-home evaluations and create personalized therapy plans for speech delays, memory issues, and safe swallowing strategies. Speech-language therapy plays a crucial role in rehabilitation and can help patients restore both confidence and function in everyday life.

Medical Social Services

Our social workers connect patients and families to critical resources—helping them navigate challenges, make care decisions, and feel supported every step of the way. From coordinating transportation and meals to offering emotional support during life transitions, our team helps patients feel empowered and informed. We also assist with advance directives, long-term care planning, and communication between medical providers and caregivers.
