What is home health care, and who qualifies?

Home health care is skilled medical care delivered in the comfort of a patient’s home. It typically includes nursing, therapy, and social work services for those recovering from illness, injury, or surgery. To qualify, patients must have a doctor’s order for care.

How is Acknowledge different from other home health agencies?

We are family-founded and family-run, with over 20 years of experience. Our patients benefit from low caseloads per manager, carefully matched field nurses, and oversight by experienced RNs. We’re also ACHC accredited and Medicare-certified.

Is Acknowledge accredited and Medicare-certified?

Yes. Acknowledge Home Health is accredited by the Accreditation Commission for Health Care (ACHC) and certified by Medicare. This means we meet high clinical and ethical standards for in-home medical care.

What services do you offer in the home?

We provide skilled nursing, IV therapy, wound care, physical therapy, occupational therapy, speech therapy, and medical social services—all tailored to each patient’s medical needs and goals. Learn more on our Services page >

What does a skilled nurse do during a visit?

Our nurses monitor health conditions, manage medications, perform wound care, start IVs, educate patients and families, and coordinate with doctors. Each visit is customized to the patient’s care plan.

Can you provide IV therapy, wound care, and physical therapy at home?

Yes. Our trained clinicians deliver advanced care in the home, including IV therapy, complex wound management, and rehabilitation services to help patients recover safely and comfortably.

How often will a nurse or therapist visit my home?

Visit frequency is based on the patient’s physician-ordered care plan. Most patients receive 1–3 visits per week, but frequency may increase or decrease depending on medical need and progress.

Do I need a doctor’s referral to begin services?

Yes. A doctor must order home health services. We can coordinate directly with physicians to streamline this process.

How do I refer a patient or loved one to Acknowledge?

Referrals can be submitted online, by phone, or by email. We accept referrals from physicians, insurance case managers, hospital discharge planners, and family members. Do you accept VA patients and work with case managers?

Absolutely. We specialize in working with VA case managers and understand the VA’s documentation and communication needs. Veterans are a vital part of our mission.

What is your referral process and timeline to begin care?

Once a referral is received and approved, we schedule an initial nurse evaluation within 24–48 hours. Care typically begins shortly after, depending on the patient’s availability and needs.

Does Medicare or Medi-Cal cover home health services?

Yes. We are a Medicare-certified and Medi-Cal-contracted provider. Most patients pay no out-of-pocket costs when services are medically necessary and physician-ordered.

Do you accept private insurance, and will there be out-of-pocket costs?

We work with many private insurance plans. Costs vary depending on coverage, but we will verify benefits and inform patients before care begins.

What benefits do you offer to field staff?

We offer benefits such as staff medical, dental, and vision insurance, paid holidays, vacation time, sick leave, CEU credits, overtime pay, and a 401(k), depending on your status and length of time with Acknowledge. Our team enjoys some of the best benefits in the industry.

How do you support nurses and therapists in the field?

We keep caseloads low, match patients thoughtfully, provide de-escalation training, offer a field safety program, and provide real-time support. Our leadership team respects and values the critical role our providers play.
